Most denied IPs remain listed because they fail to have a rDNS suitable for SMTP email servicing.
If you are running an SMTP server, you must ensure that the rDNS entry matches your server's EHLO/HELO string (while ensuring the entry is a FQDN and from a domain under your control - and not from a domain of your ISP or hosting service provider, etc.)

(EMAIL) = Email required, from IP, to confirm proper server configuration prior to removal.
(FCrDNS) = FCrDNS check failure, may require email from IP, to confirm proper server configuration prior to removal.
(FW) = Firewall is allowing internal systems direct Internet SMTP access.
(FRV) = Firewall Requires Verification - it has allowed internal systems direct Internet SMTP access.
(IP) = IP related problem - rDNS IP mismatch.
(NOMX) = Domain has no MX record.
(NXD)/(NXDOMAIN) = Non-existent domain; Invalid/unresolvable FQDN.
(OVERLOAD) = rDNS has more than one entry.
(RDNS) = rDNS issue.
(SITE) = SITE related issues.
